 The actions of a person in a manner which

vary from the accepted or legislated safe
practice and create a hazard to either
themselves, another person, or equipment.
Tindakan tidak aman/berbahaya (unsafe
action).
 Termasuk dalam tindakan tidak aman antara lain :
1. kecerobohan,
2. meninggalkan prosedur kerja,
3. tidak menggunakan alat pelindung diri (APD),
4. bekerja tanpa perintah,
5. mengabaikan instruksi kerja,
6. tidak mematuhi rambu-rambu di tempat kerja,
7. tidak melaporkan adanya kerusakan alat/mesin
ataupun APD,
8. tidak mengurus izin kerja berbahaya sebelum memulai
pekerjaan

 UNSAFE CONDITION

A condition in which something exists

that varies from a normal accepted safe
condition and, if not corrected, could
cause injury, death, or property damage.
Kondisi tidak aman (unsafe condition)
 contohnya antara lain :
1. tidak dipasang (terpasangnya) pengaman (safeguard)
pada bagian mesin yang berputar, tajam ataupun
panas,
2. terdapat instalasi kabel listrik yang kurang standar
(isolasi terkelupas, tidak rapi),
3. alat kerja/mesin/kendaraan yang kurang layak pakai,
4. tidak terdapat label pada kemasan bahan (material)
berbahaya, dan sebagainya.

Pengendalian Resiko/Bahaya dengan cara
eliminasi memiliki tingkat keefektifan,
kehandalan dan proteksi tertinggi di antara
pengendalian lainnya.
