FUEL CONTROL AND REFUEL PROCEDURE

OBJECTIVE

To provide Technicians , Supervisors, Artisans, Operators and Refuel bay attendants with
Guidelines and Procedures to be observed during replenishing of bowsers, refueling of
diesel driven machinery and disposal of old oil.

4.11.1 SURFACE SYSTEM ( IVAN AND TM3 AREA )

1. The bowsers will be filled by the Contracted Oil Company in the presence of the appointed
refuel attendant as often as is necessary.

2. The orders, receipts and waybills will be controled by the appointed mine store employee.

3. The appointed Engineering Foreman will at least monthly verify reciepts and waybills.

4. Daily measurements will be made and recorded by the appointed attendant. The attendant
will report his findings to the Engineering Foreman.

5. The appointed attendant will ensure that :-

i. No oil or fuel spillage has accumulated behind the bund walls.

ii. There is no combustible material stored or stacked near the bowsers.

iii. No persons smoke and that no naked flames are allowed within the demarcated area.

iv. The fire fighting equipment is available and that the required full fire extinguishers
are positioned in the designated areas.

v. Vehicles engines are switched off whilst they are refueling.

vi. He records the time and amount of fuel and oil issued to each specific machine.

vii. Machines are not overfilled and that hoses / funnels, fuel and oil caps etc. are replaced
prior to the machines leaving the area.

viii. Any oil / fuel spills are cleaned up or covered with sand after the machines have
left the area.

4.11.1 SURFACE SYSTEM ( IVAN AND TM3 AREA )

ix. Fuel or oil leaks observed on any machine are noted and reported to the responsible
Engineering Foreman or Technician.

x. The distribution valves at Ivan bowser on surface feeding the underground system are
positioned as per the Engineers and Foreman’s instructions.

xi. He does not leave the designated area until relieved - should relief fail to arrive he will
report to the Engineering Foreman in charge.

xii. The distribution hose nozzles are locked in position when not in use and that he retains the
keys in his possession whilst in charge of the refuel bay.

4.11.2 UNDERGROUND SYSTEM

1. Filling of diesel tanks underground will only take place on the morning shift and only as
directed by the Engineer.

2. The pipes feeding the underground system from surface through Ivan shaft will be empty
at all times except when filling is in progress. The pipe line will be thoroughly examined
fortnightly when the shaft is examined.

3. The distribution bay underground will be kept clean and no fuel or oil will be allowed to
accumulate in the area. Should this occur the appointed attendant will inform the
Engineering Foreman immediately.

4. The refuel bay underground equipment will be examined and a shift check list will
be completed for each shift by the attendants and handed in to the Engineering
Foreman. This check list will reflect :-

i. Condition of fire suppression system and strategically placed fire extinguishers.

ii. Ventilation flow and flaps.

iii. Demarcation of area and the required notice boards.

iv. Fans operational and the availability of water.

v. Telephone in working order.

vi. Cleanliness of the area (no combustible material ).

vii. Chambers locked and the keys retained by him.

viii. Oil and fuel meters, hoses and gauges in working condition (no leaks etc.).

ix. Start and end of shift meter readings.

x. Compliance to the procedures by the operators.

5. The Engineering Foreman will on a daily basis ensure that the supply of oil and fuel in
the bowsers underground is replenished as directed by the Engineer . He will ensure
that the attendants are equipped with the required P.P.E. and have the necessary equipment
to ensure that refueling and oil filling can be performed efficiently. (minimal spillage and
contamination).
4.11.3 GENERAL

1. All appointed refuel bay attendants will be trained and competent in the use of fire fighting
equipment and fire extinguishers

2. At least once per week the Engineering Foreman will over inspect the refuel systems on
surface and underground and satisfy himself that the rules applicable are being followed
and that conditions at the fuel distribution and storage sites are acceptable.

3. The Shaft Foreman will ensure that the sites underground are examined weekly and kept
safe by an appointed holder of a blasting certificate.The results of each examination will
be recorded in a book provided for this purpose.

4. All fuel storage and refuel sites will be inspected by the Shaft Foreman at least once a
fortnight and he will record the results of his findings in his log book.

5. During the “off shift” periods fuel distribution system will be locked and the keys stored as
directed by the Engineering Foreman.

6. Fuel consumption for each vehicle will be calculated on a weekly basis and anomalies
discussed at the weekly trackless meeting.

7. In order to control pollution and build up of all old / used oils and chemicals at the decline
workshops and workings the following procedures will be applied.

i. All water used to wash vehicles or work area floors will be routed by means of drains and
pipes into the specific gravity separator situated adjacent to the workshop and wash bay.

ii. Separated oil will be pumped out of the gravity separator into the old oil reservoir .

iii. All oils drained from the vehicles engines , hydraulic systems etc. will be poured into
the demarcated receipt container and pumped into the old oil reservoir.

iv. No smoking or open flames will be permitted within the demarcated reservoir and separator
area.

v. Adequate fire fighting equipment and fire extinguishers will be provided at these sites.

vi. All water passing through the gravity separator will be routed via pipes to the Rope Skimmer
(Drizit) where any addition old oil will be collected in drums. Full drums will be sealed and
transported to the salvage yard.

vii. No accumulations of old oil or oil mixtures will be permitted to remain in the underground
workings. If and when these occur the oil or mixture must be brought to surface in drums
and pumped into the old oil storage reservoir or transported to the salvage yard storage area.

viii. The appointed Engineering Foreman will ensure that : -

a. The procedures pertaining to used oil are adhered to by all Contractors, Artisans,
Technicians and Staff.

b. The old oil reservoir content is monitored daily to ensure that it is drained by the Contractors
who purchase old oil mixtures timeously.

c. Drums of old oil mixtures are not permitted to accumulate at the workshop areas. If and
when it is necessary to fill drums with such mixtures they will be transported to and stored
in the salvage yard area.

d. The oil collected at the Rope Skimmer (Drizit) is regularly removed and transported to
salvage yard in sealed drums.
