INITIATION PHASE :
1. Request for Proposal:

 The end user of the project will make a request to their


Purchasing Department, which the latter will source to
prospective suppliers/contractors for its bidding. After
evaluating the bids, Purchasing Department will award
the order to the responsible supplier who will tender
the most suitable, responsive offer.

 Please see Appendix A for bid offer or quotation by the


supplier/contractor, and Appendix B for its purchase
order.

the Application of Koncreflex Polyurethane (PU)
Coating to the 970 square meters, Refinery’s
Mixing Room Floor
 Project Cost: Php2,211,090.00
 Project Duration: Ten (10) Working Days
 Supplier/Contractor: Uptown Industrial Sales,
Incorporated
 Project Manager: Engr. Aron J. Leonoras

3. Project Objective
To comply with the ISO (International Standard
Organization) compliant requirements directed
by the BFAD (Bureau of Foods and Drugs), a
national agency regulating hygiene processes
and safety practices in consumption-based
products by using food grade and anti-microbial
material and equipment in their manufacturing
plants.

4. Project Description
Uptown Industrial Sales, Inc. will replace the
dilapidated epoxy painted floor in the refinery’s
mixing room by supplying and applying a 4 mm
thick, Koncreflex MD Polyurethane material,
cream color. This job will be accomplished within
the agreed ten (10) working days period. The
material and workmanship shall bear the warranty
of five (5) years from the date of acceptance by
the end user of the project.

7. Contracting Parties

First Farmers Holding Corporation:


 Customer is a small-size sugar mill and refinery
located in Barangay Dos Hermanas, Talisay City, in
the province of Negros Occidental.

Uptown Industrial Sales, Incorporated:


 A supplier and contractor of imported, Koncreflex
Polyurethane (PU) Industrial Flooring System, with
head office at Mandaluyong City in Metro Manila, and
branch office located in # 7 RBM Building, Araneta
Street, Bacolod City.

8. Project Contract
a. Project Warranty:
Uptown offers a single warranty covering both
materials and workmanship for a period of five (5)
years commencing from the date of acceptance.
Coverage of warranty is limited to delamination
and/or peeling off resulting from normal use of the
floor based on what it was originally designed for.
b. Terms of Payment: Thirty (30) percent down
payment, balance upon completion.
c. General Terms and Conditions: Please see
Appendix C.

PLANNING PHASE :
1. Project Requirement: Nine hundred seventy (970)
square meters of floor area to be PU coated.
2. Scope of Works: Please see Appendix D.
3. Cost of the Project: Php2,211,090.00, VAT

ed by client, First Farmers Holding
Corporation.

EXECUTING PHASE:
1. The Project Manager will coordinate working
schedules and protocols with the Purchasing
Department for the clearances of personnel who
will work in the area, and with the end user who
will provide the needs of electricity and water.

Likewise, he will secure the food and lodging


provisions of Uptown Personnel. He will also
liaise with the Purchasing and Refinery
Departments for the updates, completion and
acceptance of the project.

2. The Project Foreman will ensure the daily
implementation of project activities with
optimum utilization of materials and man-
hours vis-à-vis project schedules and
requirements.
3. The Coating Specialist will ensure the proper
application of Koncreflex MD PU in
accordance with customer’s specifications of 4
mm thickness.

4. The helpers will provide the general labor
as maybe directed by the Project
Foreman and Coating Specialist.

Likewise, assessment of daily project


execution, events, and important related
information shall be done by key
stakeholders of Uptown Industrial Sales,
Inc.

MONITORING AND CONTROL
1. Measuring the ongoing project: Planned Activity
vs. Actual: Please see Appendix F.
2. Critical Path Method (CPM) of the project:
Please see Appendix G.
Should there be any deviations in the actual
progress of works vis-à-vis the planned schedules,
the Project Manager or Project Foreman may call
for an overtime work in order to expedite the
delayed activities

CLOSING PHASE
1. Contract Closure:
a. Customer’s checking of applied area in terms
of workmanship and conformity to required
specifications. In accepting the outcome of
the project, authorized end user will
acknowledge the Delivery Receipt (DR) and
Sales Invoice (SI) of Uptown Industrial
Sales, Incorporated.

b. Demobilization of Uptown Personnel,
including securing of gate pass
clearance for withdrawal of tools and
equipment from mill premises.

2. Project Closure:
a. Uptown, thru its Project Manager,
Engr. Aron Leonoras will compen-
sate the labor services of its hired
contractual workers.
b. Project Manager, Engr. Aron
Leonoras will submit billing state-
ment to First Farmers in order to process its
seventy percent (70%) balance payment.
c. Project Manager, Engr. Aron Leonoras will
collect the First Farmers’ payment check.
