Root Cause Problem Solving Sheet

TOPIC: Participants: Last update: Step 4: Propose the initiative / solution for the identified root cause
Score 1: Low impact/difficult to implement -------------- Score 4: High impact/easy to implement

Proposed solution Easy imple. Impact Priority Comment

Step 1: Define problem statement

What:
(What is exactly the problem we want to address?)

Where:
(At which detailed location did the problem occur in the flow? Where in the machine/area? During what operation? Which component?)

When: How Many/Much?

(When did the problem occur? In what period? After any specific event?) (Frequency of incidents? How much deviations to target? Etc..)

Who:
(Who is affected by the problem? Who detected this problem? Who could help on this problem?)

Why/Impact:
(Why it is a problem and try to quantify the impact on cost, lost volume, lost time, KPI?)

Step 2: Identify/prioritize main causes- (use fishbone or flow process diagram) Step 5: Plan the improvement actions to implement the selected solution

Actions Who By when Status Comments

Step 3: Analysis on process(es) and factor(s) that might cause the problem Priority Exclude, not a priority

Conclusion
of problem Why? Why? Why? Why? Why?
statement

Step 6: Propose the measurement method with its frequency and specific KPIs (Include Leading and
Lagging KPI)

Step 7: Plan and generate the standardization initiatives and share the solution across lines / sites
(create a standard, report, checklist. etc.)
