Prastika Agustina Damayanti (173210031) Nursing diagnoses are clinical decisions about individual, family, or community responses to actual and potential health problems, or life processes.(NANDA International,2007). Nursing diagnosis is a statement that explains the human response (health status or risk of pattern changes) from individuals or groups in which the nurse can accountably identify and provide interventions to maintain health status, limit, prevent, or change it.(Carpenito,2000). The formulation of a nursing diagnosis is based on identifying the client's needs. When the assessment data begins to show a problem, the nurse is directed to the selection of appropriate nursing diagnoses. Nursing diagnoses according to Carpentio (2000) can be divided into 5 categories: 1. Actual 2. Risk 3. Possibility 4. Nursing wellness 5. Nursing Syndrome 1. Domain 1 health promotion 2. Domain 2 nutrition 3. Domain 3 elimination and exchange 4. Domain 4 activity – rest 5. Domain 5 of perception – cognition 6. Domain 6 self-perception 7. Domain 7 role relationship 8. Domain 8 sexuality 9. Domain 9 coping / stress tolerance 10. Domain 10 principles of life 11. Domain 11 security / protection 12. Domain 12 comfort 13. Domain 13 growth / development Nursing diagnoses focus on and identify the nursing needs of the client (Gordon, 1994). Nursing diagnoses reflect the level of health or response to disease or pathological processes, emotional status, sociocultural phenomena, or stages of development. Medical diagnoses prominently identify the status of specific diseases. The medical focus is on the diagnosis and treatment of diseases. Medical diagnoses identify specific disease status. Medical focus is on diagnosis or treatment of disease. Medical diagnosis and nursing diagnoses are developed using assessment data bases. However, basic nursing data is global, and includes an in-depth assessment of the physiological, psychological, socio-cultural, developmental and spiritual client. Prioritizing a nursing diagnosis should be sorted according to the client's main circumstances and needs, with categories: 1. Based on the level of gravity a. Life threatening situation. b. Circumstances that are not serious and not life threatening. \ c. Perceptions about health and nursing. 2. Based on Maslow's needs, namely physiological needs, security and safety needs, loving and loved needs, self-esteem needs and self-actualization needs. 3.
