Definition

A record of information about a patient's


drug therapy.

The profile can contribute to better patient


care by enhancing the pharmacist's ability
to efficiently perform his professional
duties

Definition

The intent of the PMP is to consolidate


pertinent facts relating to a patient's drug
therapy such as-name, age, sex, weight,
height, allergies or drug sensitivities, and
chronic disease conditions, plus all
prescription and o-t-c medications being
used.

These facts must be arranged so that the


pharmacist can quickly render a
professional judgment on potential
problems with a patient's drug regimen.

Significance

To provide a history of patient’s previous


medication including a note of
idiosyncrasy, adverse drug reaction,
allergies together with a record of his
current medication.

This enables the practitioners, and


pharmacist to monitor patient’s
medication.

An analysis of the profile should be


completed by the pharmacist prior to
dispensing of renewing each
prescription.

Because of the busy schedules of most


pharmacists, it is important to establish
routines that maximize the efficiency of
providing drug monitoring functions for
both new and renewal prescriptions.

With each new prescription six factors


should be systematically monitored on the
prescription and the PMP-

1. Proper dosage strength


2. Proper administration time and schedule
3. Potential drug-drug interactions
4. History of drug sensitivities or allergies
5. Duplication of an active prescription
with another for the same drug, or a drug
with a similar therapeutic effect
6. Potential disease-drug interactions

When a prescription is brought in for


renewal and the previous questions have
been properly evaluated initially, it is
generally not necessary to repeat all the
monitoring steps.

However, there are factors that apply


specifically to renewals that should be
routinely reviewed. These are-
1. Over-utilization
2. Under-utilization
3. Inappropriate length of therapy
