Desarrollo
Cadera y acetbulo desarrollan a partir de cartlago mesenquimal 7 sem.: primeros indicios 11 sem.: estruct. cartilaginosas cabeza fmur, acetbulo desarrollan separadas CPO fmur desarrolla en de difisis, crece proximal y distal a lo largo del eje. RN, el fmur proximal contiene los trocnteres > y <, la cabeza femoral Osificacin del fmur proximal: 4-6m edad. Ncleo de osificacin: visibles en US 2-3m Centro osificacin de cabeza femoral se expande, se vuelve hemiesfrica, la placa de crecimiento (entre cabeza femoral y metfisis) a los Rx se ve radiolcida

Diagnstico y presentacin clnica Vara de acuerdo a la edad del nio:
RN a 3m Prueba de Ortolani - Barlow De los 3 a 6 meses de edad Signo de Galeazzi: Paciente DCS: caderas y rodillas flexionadas (+) : una rodilla esta ms abajo que la otra Asimetra de pliegues de cara interna de muslos Paciente DCS: caderas y rodillas extendidas. (+) : pliegues del muslo son asimtricos Movimiento limitado (+): disminucin de abduccin de cadera displsica
Observando al paciente desde atrs, pdale que se mantenga sobre un pie y despus sobre el otro. En un individuo normal, la nalga del lado que se levanta del suelo se elevar debido a contraccin de abductores en lado opuesto. Test de Trendelenburg (-) Si abductores no funcionan adecuadamente, la nalga del lado que se eleva del suelo tiende a caer, y el paciente mantiene su equilibrio inclinando la parte superior de su cuerpo lateralmente sobre el lado afecto. Test de Trendelenburg (+)

ENFERMEDAD DE LEGG-CALVE-PERTHES Patogenia 1fase incipiente o de sinovitis: Cambios patolgicos afectan a tejidos blandos
Membrana sinovial y cpsula tumefactos, edematosos e hipermicos Produce derrame articular dura entre 1y 3 semanas.
2 fase se produce:
Necrosis sea avascular de 1/2 anterior o todo CPO de la cabeza femoral Hueso y mdula sea se necrosan Se destruyen trabculas seas y constituye masa de partculas de hueso y mdula muertos El hueso sufre aplastamiento y colapso progresivo No hay regeneracin sea. 6 a 15 meses de la interrupcin vascular, dura entre varios meses y un ao.

ENFERMEDAD DE LEGG-CALVE-PERTHES Patogenia La tercera fase se caracteriza:
Fragmentacin y regeneracin de cabeza femoral Tejido conectivo, vascular y celular invade el hueso muerto El cual es reabsorbido y restituido por hueso inmaduro de formacin reciente. Se produce fragmentacin de secuestros seos y reparacin vascular. Dura entre uno y tres aos.
4 fase
Reparacin, crecimiento y reconstruccin, restablece la estructura sea normal. Hueso normal sustituye al hueso muerto Se produce remineralizacin de zona necrosada Evolucin favorable finalizar con la congruencia articular entre cabeza femoral y acetbulo, (depender de edad, grado de afectacin de cabeza femoral, tratamiento)

ENFERMEDAD DE LEGG-CALVE-PERTHES Estadio final residual: Ensanchamiento cuello femoral, cabeza femoral aplanada y ensanchada Crecimiento adaptativo de cavidad acetabular Articulacin de aspecto voluminoso, pudiendo alcanzar una buena congruencia articular. A veces la cabeza femoral aplanada y ensanchada protruye ms all del reborde acetabular, con incongruencia anatmica entre cabeza femoral y acetbulo, (en vida adulta generar cambios degenerativos y artrosis tarda)

Cuadro clnico
Sntomas:
comienzo insidioso dolor y/o claudicacin progresivo de varios meses de evolucin A veces debut brusco, puede estar ausente Distribucin sensitiva de inervacin capsular cadera (femoral, obturador y citico)
Signos:
cojera aumenta con ejercicio, empeora a lo largo del da y mejora con reposo Es clsica la presentacin en forma de cojera indolora. Talla baja

SINOVITIS TRANSITORIA Clnica:
Nio se despierta por la maana con cojera y rechazo a caminar Despus, puede mejorar, pero empeora al final del da Nio localiza dolor en parte anterior del muslo proximal EC: restriccin leve del movimiento de cadera, abduccin es la + afectada (20 40)
Diagnstico: por exclusin: LCP, infecciosa, trauma, DFP, AR, tumores US es la tcnica de imagen de eleccin en el diagnstico Tx: reposo en cama Evolucin: Normalmente se resuelve en 2 semanas RX de seguimiento es necesaria si sntomas regresan o no a resolver sinovitis transitoria recurrente es asociado con ELCP (2-3% de casos) Dx Diferencial: artritis sptica

Transaxial CT images of the right hip (A) show the head to be posteriorly rotated in the acetabulum, with the neck anteriorly subluxed and externally rotated on the head. Coronal (B) and sagittal (C) images show the infenor (B) and posterior (C) displacement of the head, with the neck nearly impinging on the lateral acetabular rim. The absence of healing or bridging bone is consistent with the apparently acute onset of symptoms. Careful questioning elicited a history of subtle left hip pain compatible with chronic SCFE on that side.
