definition
Sve implies to cleaning ,shaping and disinfection of the root canal system followed by obturation of root canal at the same appointmnt.

Common factors which appear to be responsible for not performng sve:-doubt of postop pain -fear of failure. -Pt. discomfort as he /she has 2 keep d mouth open 4 a long time. -lack of time. Lack of skill and equipmnt.

Advantages-WTF,PET,CAF
1. 2. 3. 4. 5. 6. 7. Lesser errors in working length. Saves time. Familiarity of the canal anatomy. Reduced intra-appointment pain. Economics. Reduces incomplete t/t. Pt. comfort-reduced number of visits , no discomfort of repetitive LA ,t/t procedure , etc. 8. Aesthetics-for pts. Who report to clinic with fractured anteriors,an aesthetic restoration can be placed immediately after SVE. 9. Minimizes the fear and anxiety.

DISADVANTAGES-TIFC
1. Tiring for the pt. 2. SVE is difficult for Inexperienced clinician. 3. Flare ups-if flare up occurs it is difficult 2 remove filling mat in an obturated canal. 4. Not possible in all cases-calcified canals,severe curvatures,weeping canals,multiple canals.

CRITERIA FOR SVE-THE PASS AC CVC


1. 2. 3. 4. Clinical experience-endo competence of the clinician Pt.cooperation. Accessibility-ant r more accessible than posteriors. Anatomic variations- teeth that have anatomic aberrations require more time to treat. 5. Calcified canals-takes more time to treat. 6. Curved canals- severely curved and tortuous canals require more time to treat. 7. Pulp status- vital teeth have fewer post-op flare ups. 8. Teeth with apical lesion , which have been left open for a long time , are not ideal for SVE. 9. Clinical symptoms-SVE should not b done in acute alveolar abscess cases. 10. Presence of sinus tract-good cases for SVE. 11. Availability of sufficient time to complete the case. 12. Absence of continuous hemorrhage or exudation.

INDICATIONS
Uncomplicated vital teeth. Physically compromised pts. medically compromised pts who require antibiotic prophylaxis. Fractured anteriors where aesthetics is a concern. Apprehensive but cooperative pts. Pts. Who require sedation. Uncomplicated non-vital teeth with sinus tract.

CONTRAINDICATIONS
Acute alveolar abscess cases with pus discharge. Pts. Who hv acute apical periodontitis with severe pain on percussion. Painful non-vital tooth with no sinus tract. Asymptomatic teeth with apical lesion and no sinus tract. Cases with procedural difficulties such as calcified , curvatures,extra canals. Pts. With TMJ disorders. Teeth with limited access. Non-surgical retreatmnt cases.
