The system was designed to compliment the public health service MIS, especially on maternal and child health indicators.

FLOW OF PROCESS
2nd During the course of pregnancy all of medical data have been noted in form 2 1st ASHA workers contacts to beneficiary & than quotes the details in form 1 3rd All the delivery related details noted in form 3
6th at last this all data entered in the software which is called CMIS 5th ORW take this data from ASHA workers submit it to branch office
4th All the postnatal data related to mother & child noted in form 4

IN MIS WORK AFTER CROSS CHECK 531 ENTRIES MATCH OUT OF 1133 ENTRIES WHICH RATIO IS ONLY ABOUT 46.86%
Hand writing was not readable. Most of registers were without title page which including name of village, cluster no. & name of PHC, sub center. Pages in registers were not in page no. wise. There were many tick mark in registers so very difficult to verify last mark and count it. Many torn pages in registers. Some names are repeated & some are check out.
Beneficiary was not meet as she was at her fathers house.(piyar) Asha replacement
