• Kelenjar prostat normal sekitar 20 g volume,

panjang 3 cm, lebar 4 cm, dan kedalaman 2
cm
• Kelenjar ini terletak posterior dari simfisis pubis
 • Hiperplasia
• Disfungsi otot detrussor, trabekula VU
Definisi • diagnosis histologis yang mengacu pada
proliferasi otot polos dan sel epitel dalam
zona transisi prostat

• 8 % ->31-40 thn
• 40-50% -> 50-60 thn
Epidemiologi • >80% -> ^80 thn

• Adenokarsinoma prostat,

• Penanda spesifik:
• PSA, (3 pengukuran lebih dari 2 tahun).
1. PSA:> 10 ng / ml (karsinoma tidak mungkin terbatas
pada organ) - prostat bx.
2. PSA: 4–10 ng / ml (karsinoma mungkin terbatas
pada organ) - prostat bx.
3. PSA: <4 ng / ml (hanya 20% akan menderita
kanker) - ± prostatic bx.

Transrectal ultrasonography (TRUS) dan biopi prostat
• Tindakan biopsi prostat sebaiknya ditentukan berdasarkan
kadar PSA, kecurigaan pada pemeriksaan colok dubur atau
temuan
• metastasis yang diduga dari kanker prostat. Sangat dianjurkan
bila biopsi prostat dengan guided TRUS,

Biopsi Ulang
Indikasi Biopsi Ulang :
• PSA yang meningkat dan atau menetap
• Kecurigaan dari colok dubur
• Proliferasi sel asinar kecil yang atipik (ASAP)
• High Grade Prostatic intraepithelial (PIN) lebih dari satu core31
• Penentuan waktu yang optimal untuk biopsi ulang adalah
• 3-6 bulan

• Kelompok resiko tinggi adalah T3a atau Score Gleason 8 s/d 10, atau
• PSA > 20 mg/mL. Diberikan terapi inisial dengan:
1. External Beam Radiotherapy (EBRT) + Androgen Deprivated Therapy (
ADT) selama 2-3 tahun (Kategori 1) atau
2. EBRT + Brakhiterapi ± ADT (2-3 tahun) dengan monitoring PSA setiap 6-12
bulan selama 5 tahun. Atau
3. Radical Prostatectomy (RP) + Pelvic Lymph Node Dissection (PLND)
