Prinzipien:
• Diagnose wird primär klinisch gestellt
• Apparative Zusatzuntersuchungen zum Ausschluss
Differentialdiagnosen
• Mögliches Fehlen von Stauungszeichen im Röntgen-Thorax
• Bestimmung von BNP bei Intensiv-Patienten mit niedriger Spezifität
Ziele:
• Stabilisierung des Patienten
• Wiederherstellung der Oxygenierung
• Verbesserung von Hämodynamik und Organperfusion (insb. Herz und
Niere)
• Minimierung der Intensivaufenthaltsdauer

Lungenembolie
Anticoagulation:
• venous thromboembolism (VTE) prophylaxis
with LMWH is recommended for all
hospitalized patients with COVID-19
infection. Withhold VTE prophylaxis for active
bleeding, low platelet count or low fibrinogen
− use fondaparinux in patients with a
history of heparin-induced
thrombocytopenia
− use unfractionated heparin
subcutaneously 2 or 3 times daily or
reduced-dose LMWH in patients with
creatinine clearance less than 30
mL/minute
− use mechanical thromboprophylaxis in
patients where anticoagulants are
contraindicated or unavailable
• therapeutic-intensity anticoagulation is not
recommended in the management of
COVID-19 in the absence of confirmed VTE
